Required Forms for Student Registration
Please print, complete, and submit the applicable forms to the Preschool
along with your initial deposit for the coming year/semester.
The forms identified at the left must be completed, or considered,
for each child being registered to attend the Pre-School.
Student Application -- basic identifying data about the child and parents
State Mandated Forms #1214 and #1215 -- must be provided for each child

Emergency Notification & Action -- provides permission to seek medical assistance
No Photographs -- of your children on our website.
The top 3 forms are mandatory and the last 2 are optional.
